Liability Waiver

I/We understand and agree that neither Northwest Ballet of Steamboat Springs, Heidi Meshurel-Jolly, as director and sole proprietor, nor any volunteers or substitute teachers shall be held responsible or made subject of any claim seeking to assess damages or liability for or arising from personal injury or property damage or loss of any other sort to myself/ourselves or our child/children in whose behalf this waiver is now signed as a result of actual or proposed participation in any Northwest Ballet of Steamboat Springs classes. I/We recognize the risks of injury inherent in any dance program and also recognize that certain mid-torso contact is necessary for alignment and correction purposes. I/We herby waive and release Northwest Ballet of Steamboat Springs from and against any and all claims, except for injury directly resulting from gross negligence or willful misconduct on the part of Northwest Ballet of Steamboat Springs.
